A first win over Sassuolo at the third time of asking is enough to see Milan crowned champions of Italy - winning the Scudetto with 86 points; two ahead of rivals Inter to seal their first victory since 2010-11. The job was done at half-time for the Rossoneri, who quickly accumulated an unassailable 3-0 lead thanks to a double from Giroud and a farewell strike from Kessie; all assisted by Leao. The sting was taken out of the match after the restart with the Neroverdi looking to limit the damage and Milan happy to see out the final 45 minutes in comfort en-route to a title win. The convincing performance seals Milan’s 19th Scudetto - a first title for Pioli - while Sassuolo finish the year in 10th place.

That concludes our commentary of the Serie A fixture between Sassuolo and Milan! We hope you enjoyed it - as well as the season as a whole. Thank you!

  11. 78'

IBRAHIMOVIC SCORES BUT IT’S OFFSIDE! A pass is launched over the top for Leao, who drives towards the byline before sending a cross to the far post for Ibrahimovic. The veteran striker leans forward and nods the ball past Consigli but the linesman’s flag goes up immediately as the Portugal international was unable to keep onside.19:43

  12. 76'

Raspadori takes a Sassuolo corner but Maignan gets two assured hands to the ball, calling off Diaz in order to make a clean catch.19:41

  13. 74'

Milan will look to play keep-away for the final 16 minutes, forcing Sassuolo to chase the ball as time expires. Down by three goals and forced into nothing more than speculative strikes from long range, the Neroverdi will no doubt be waiting for the final whistle.19:39

  14. 73'

Krunic is also given a rest, with Diaz sent into the fray for the remainder of the match.22:55

  15. 72'

The referee once again blows his whistle and points to the touchline for the second-half cooling break. It’s one last chance for Pioli to give his side instructions to see out the remainder of the match.19:36

  17. 70'

Milan have picked up 83 points in Serie A this season, their highest total in the three points for a win era (excluding 2005/06 when they had a deduction). The Rossoneri have won 25 league games so far; only three times previously have they won at least 26 games in a top-flight campaign: 2005/06, 1950/51 and 1949/50.19:34

  18. 67'

An injury to Berardi forces him to depart from the match after 68 minutes. The Italy international's season comes to an end - to be replaced by Defrel.20:55

  19. 66'

Saelemaekers swings a corner into the penalty area from the right wing which is caught easily at the far post by Consigli. The referee blows his whistle as the ball had gone out of play before coming back in - giving the Sassuolo goalkeeper a free-kick.19:30

  20. 64'

The intensity of the match has dropped somewhat, with Sassuolo looking to limit the damage and nick something on the counter-attack while Milan control the tempo and look to see the game out with minimal fuss. 19:28

  21. 62'

Giroud stumbles into the penalty area and looks to tease a cross to the far post for Saelemaekers but Ferrari puts himself in position to get to the ball first and clear the danger.19:27

  22. 60'

Hernandez puts a low cross towards the penalty spot for Giroud, who unselfishly attempts to lay it off for Leao instead of having a strike on goal himself. A miscommunication ends the move however as Sassuolo manage to scramble it clear.19:25

  23. 58'

Traore comes on for Frattesi with just over half an hour remaining in the contest.19:23

  24. 57'

A second goal in quick succession for Correa has given Inter a 3-0 lead, mirroring the score between Milan and Sassuolo.19:22

  25. 55'

Inter have gone ahead by two goals in their match against Sampdoria, with Joaquin Correa adding to Ivan Perisic’s earlier effort. Both Milan clubs are doing their best to end the season on a high but only one will come out on the other end with the Scudetto.19:21

  26. 53'

Kyriakopoulos - only just back from suspension - is shown a yellow card for a cynical foul in midfield on Saelemaekers.19:18

  27. 52'

News from the Giuseppe Meazza, as Perisic has given Inter the lead after 50 minutes. Even with that goal though the Nerazzurri will need a lot of help from Alessio Dionisi’s side; who need to overturn this three-goal deficit in order to give Inter a chance to be champions.19:16

  28. 50'

Henrique glides forward through midfield before finding Berardi on the edge of the penalty area. The winger looks to put in a cross which is blocked at the outset by Hernandez before cutting inside and firing a shot that sails high and wide.19:14

  29. 48'

Bennacer gives the ball away to Scamacca but his powerful pass forward is intercepted by Kalulu - sending Milan away on an attack of their own.19:12

  30. 46'

Sassuolo get the match restarted from the kick-off!19:10

  31. 46'

Magnanelli comes on at the half for Lopez.19:10

  32. 46'

A change at half-time sees the booked Tonali replaced by Bennacer.19:10

  33. Sassuolo were in disarray in the first half, giving the ball away in poor situations which led to Milan’s first two goals. The experienced Chiriches or someone to hold onto possession - Djuricic perhaps - could do well to limit the damage going forward. As for Milan, with just 45 minutes remaining they’ll keep firmly focused on keeping play in front of them. Pioli may choose to give some lesser-seen players gametime as well, with Gabbia and Ballo-Toure in line.18:55

  34. A perfect first half for Milan comes to a close at the MAPEI Stadium, with the Rossoneri leading Sassuolo by a deserved three goals to nil. Pioli’s side started with intensity both offensively and when out of possession and took just 17 minutes to find the breakthrough, when Leao’s cutback found the foot of Giroud, who swept it home. Milan employed the same tactic 15 minutes later with similar results - with the Portugal international playing it back to the former Chelsea striker, who would add a second to his collection. Kessie’s farewell goal just 240 seconds later capped an extremely impressive 45 minutes for the potential champions, while Sassuolo offered precious little to stand in the way of Milan’s title charge. 18:55

  35. 45'+2'

    HALF-TIME: SASSUOLO 0-3 MILAN.18:54

  36. 45'+2'

The Inter and Sampdoria match has gone to the half-time interval, with the two sides level at 0-0; adding a little bit of extra security to Milan's title bid.18:52

  37. 45'

The fourth official indicates three minutes will be added to the end of the half for stoppages.18:51

  38. 43'

GREAT SAVE! Raspadori leaves the free-kick for Berardi, who finds the head of Frattesi on the edge of the 18-yard box. The backwards header looks extremely difficult to handle but Maignan drops low to make a crucial one-handed stop.18:48

  39. 42'

Leao lunges in and takes Frattesi down, giving Sassuolo a free-kick on the right wing in a similar position to their earlier set-piece.18:47

  40. 41'

With just under five minutes to go in the first half, it’s been about as perfect an opening period as Pioli could have hoped. Hungry for goals in attack and desperate to defend their own goal, the Rossoneri have played with intent and desire from the opening whistle - and they are more than deserving of their three-goal lead.18:45

  41. 39'

Saelemaekers swings a cross into the penalty area from the right, which is met by a glancing header by Giroud. The striker fails to secure his hat-trick however as Consigli meets the shot with two secure hands.18:43

  42. 37'

A frustrated tackle by Lopez on Krunic earns the Sassuolo midfielder a yellow card.18:42

  43. 36'

It's a hat-trick of assists for Leao in 36 minutes, with the Portugal international again cutting a pass back for a team-mate; with Kessie on the end of it this time.18:42

  44. 36'

IT’S A FAREWELL GOAL FOR KESSIE! 3-0! In his last match for Milan before a summer transfer to Barcelona, the Ivory Coast international gives Milan a three-goal cushion - latching onto Leao’s cutback pass and hitting a powerful first-time effort with the inside of his foot!

Tonali goes through the back of Berardi, bringing the Italy international to the turf. It's a yellow card for Tonali, who will need to take a breather to collect himself.18:39

  46. 32'

Leao makes it two assists today with a similar effort to his earlier supplying pass for Giroud.18:39

  47. 32'

GIROUD MAKES IT TWO! 2-0! Another cutback, another goal for Milan after Ferrari loses possession inside his own penalty area to Leao before forcing the winger out of the 18-yard box. The Portugal international then beats his man while driving to the byline and passes it towards the penalty spot for Giroud - whose simple finish beats Consigli!

Raspadori is tasked with the free-kick but Ayhan is unable to hit the target from his close-range corner.18:35

  49. 30'

Berardi wins a free-kick after a physical duel with Kessie - giving Sassuolo a chance to put the ball into Milan’s penalty area with a set-piece delivery from the right wing.18:34

  50. 29'

In 17 Serie A matches between Sassuolo and Milan, the Rossoneri have won nine games, with the other meetings seeing two draws and six Neroverdi wins (including both of the last two meetings).18:33

  51. 27'

The referee blows his whistle and points to the touchline, with a cooling break putting a pause in the action. It’s been a breathless opening phase of the match at the MAPEI Stadium, with relentless attacking from both sides. 18:31

  52. 25'

The match at the Giuseppe Meazza between Inter and Sampdoria remains 0-0 after 25 minutes, which has opened up a four-point gap between the two Milanese clubs at the top of the table. With just over an hour left in both fixtures, there’s still a lot of time to play - and a lot to play for!18:29

  53. 23'

WHAT A HIT! Saelemaekers looks to drive into the penalty area in a central position but with two defenders in his way, the winger opts to lay it off for Tonali, who latches onto the ball and smashes a powerful hit that forces a diving save out of Consigli!18:27

  54. 21'

Saelemaekers takes the corner short to Kessie, who immediately passes it back to the winger but the eventual cross is easily cleared away.18:25

  55. 19'

Calabria puts a low cross into the penalty area for Giroud immediately after the goal looking to add a bit of insurance but Ferrari does just enough to turn the ball behind for a corner.18:24

  56. 17'

GIROUD GIVES MILAN THE LEAD! 1-0 TO THE ROSSONERI! Ayhan is caught in-possession on the halfway line, eventually ceeding the ball to Leao. The winger then bursts forward and gets into the penalty area before cutting it back to Giroud. The striker takes his shot on the swivel and manages to squeak it underneath Consigli to take the lead!

Hernandez’s latest offering from a corner-kick is sent right into the hands of Consigli, who holds onto the ball expertly.18:20

  58. 14'

Hernandez delivers a cross into the penalty area from the left but the full-back’s ball is put a half-step ahead of the arriving Giroud, who fails to get on the end of it.18:19

  59. 12'

Milan have done everything but score in the first 10 minutes, with the Rossoneri looking to take the match by the scruff of the neck. With destiny in their own hands the last thing Pioli will want is to be forced into watching scores from elsewhere.18:16

  60. 10'

TOMORI CAN’T FINISH! Hernandez takes over corner-kick duties, sending his set-piece to the far post for Tomori. The centre-back looks to have half of the net to aim at but Lopez does just enough to clear the ensuing shot off the line!18:14

  61. 8'

WHAT A CHANCE! It’s the first opportunity of the match as Saelemaekers swings in the free-kick, finding the head of an unmarked Giroud. The striker redirects the ball towards goal with his head - forcing Consigli into a brilliantly athletic save!18:12

  62. 7'

Henrique brings Hernandez down with an outstretched leg - giving Milan a free-kick 35 yards from goal.18:11

  63. 6'

Saelemaekers is beaten to the loose ball by Kyriakopoulos, who finds Berardi with a simple pass. The Italy international cuts inside and looks to beat Maignan at his far post but the shot fails to hit the target.18:10

  64. 4'

Milan have started the match with very high intensity, driving forward at every opportunity while pressing their opponents when out of possession. Pioli’s side are looking to find an early goal to settle the nerves, which could open spaces in behind.18:08

  65. 2'

Leao takes an early Milan corner short but a loose pass back to the winger gives Sassuolo a throw-in.18:06

  66. 1'

Milan get the match underway from the kick-off!18:05

  67. Milan name an unchanged XI from the side that won 2-0 against Atalanta seven days ago. Maignan starts behind a back four of Calabria, Kalulu, Tomori and Hernandez, while Tonali and Kessie are tasked with sitting just ahead of the defence. Saelemaekers and Leao are the chosen wingers with Krunic playing just behind striker Giroud. 17:51

  68. Sassuolo make three changes to the XI that beat Bologna 3-1 last Sunday. Kyriakopoulos replaces Rogerio at left-back while Ayhan is in for Chiriches in defence. The final alteration is in midfield, where Traore is dropped for Lopez.17:51

  69. MILAN SUBS: Junior Messias, Ismael Bennacer, Alessandro Florenzi, Brahim Diaz, Ciprian Tatarusanu, Alessio Romagnoli, Ante Rebic, Antonio Mirante, Fode Ballo-Toure, Matteo Gabbia, Tiemoue Bakayoko, Zlatan Ibrahimovic.17:46

  70. MILAN XI (4-2-3-1): Mike Maignan; Davide Calabria(c) Pierre Kalulu, Fikayo Tomori, Theo Hernandez; Sandro Tonali, Franck Kessie; Alexis Saelemaekers, Rade Krunic, Rafael Leao; Olivier Giroud.17:46

  72. SASSUOLO XI (4-2-3-1): Andrea Consigli; Georgios Kyriakopoulos, Gian Marco Ferrari(c), Kaan Ayhan, Mert Muldur; Matheus Henrique, Maxime Lopez, Davide Frattesi; Giacomo Raspadori, Gianluca Scamacca, Domenico Berardi.17:46

  73. With 15 minutes to go before kick-off, let’s take a look at how the two sides are lining up; starting with our hosts!17:46

  74. After 37 matchweeks, it all comes down to this for Milan, who are one point away from their first Scudetto since the 2010-11 season. A run of five-consecutive league victories including three clean sheets has given Stefano Pioli’s side the edge over Inter with 90 minutes remaining in the year, and two points separating the Milanese clubs. Any dropped points by the Nerazzurri in their match against Sampdoria would see the Rossoneri crowned champions - as would a win or a draw for Milan at the MAPEI Stadium. 20:10

  75. Three points behind Hellas Verona but having lost the season’s head-to-head contest, Sassuolo are destined to end the season right in the middle of the Serie A table with a 10th-place finish. That doesn’t mean there is nothing left to play for however, as the Neroverdi are aiming to assume the role of spoiler; looking to end Milan’s title bid at the final hurdle with a third-successive victory over the Rossoneri.17:29
