Juve were poor after leading through Higuain, seeing Kalinic equalise before Perotti's brace - the first from the penalty spot, the second deflected - settled the match.
Both sides' rested stars will return next week as the focus turns to Europe. Juve have that big clash with Lyon, while Roma face Sevilla in the Europa League.
